Extended Care Facility/Nursing Home   

Occasionally, a critically ill or injured patient has not recovered enough to participate in a rehabilitation program, a skilled nursing program, or to be safely cared for at home. The decision to move a loved one to a nursing home is one of the most difficult and emotionally draining decisions a family can make. Usually all other options have been exhausted. Moving a loved one to the nursing home may be a permanent or temporary placement.

The nursing home provides twenty-four a day care for the patient, assisting them with the activities of daily living. The goal of most nursing homes is to provide care and treatment to restore or to maintain the patient's highest possible level of physical, mental, and social well being.

The medical staff provides guidance for the patient and family regarding their best options. The social worker and case manager (if applicable) may be the best resource for information.

Families should visit the nursing home prior to making a decision to move a loved one and consider the following aspects:

 

red dot  The location. This is important regarding the accessibility for visiting.

red dot  The staffing. Use common sense. Is the staff friendly to you and the residents? Is there a high turnover of staff and if so, why? Does the staff respond to the resident call lights in a reasonable fashion?

red dot  What is the availability of beds?

red dot  If the resident needs to transfer to the hospital, how long will their bed be held?

red dot  How does the patient room selection occur?

red dot  What are the visiting hours?

red dot  Visit the nursing home prior to moving your loved one. Take the tour and if possible, visit once after the tour to observe routine activities.

red dot  Ask for the institution's most recent inspection results.

red dot  What is the food like?

red dot  Are there any special needs involved in the care of your loved one-religious, language, dialysis, etc. and can they be met?

red dot  Who will be providing the care to your loved one?

red dot  How does the doctors' coverage work?

red dot  Word of mouth references can be very important. Speak to your family and friends. Also ask visitors of current residents what they think of the facility.

red dot  Be sure that you understand the cost of the nursing home including any extra fees that might be required, such as fees for activities in which the patient may participate.

red dot  Does the nursing home utilize volunteers and do they allow pets?

red dot  Does the nursing home allow the resident to have a few personal items?
